New Patient instructions
1. All MMP Patients are required to complete all medical history & information in our Electronic Health Record System to register, you must also download and complete our registration form prior to your office visit.
2. We need supporting documentation on your medical health history so request your medical records from the last year from your primary care physician and any specialists as soon as possible.

($100 New Patient Deposit Required)
Protocol for MMP Patients
Initial Consultation & Blood work $400
Follow Up for Physical Examination $300 (Two weeks after Initial Consultation)
3 Month Wellness Follow Up $200
4. Bring with you all vitamins, minerals, supplements, and prescription drugs you may be taking.
5. For patients who will be filing for reimbursements go online to your insurance provider’s website and print out a claim form, If you do not have access to a computer make sure to call your insurance company and have them send you a claim form. All claim forms will be completed during your office visit to expedite reimbursements.
6. Know the Law DC Department of Health MMP
TIMM is a fee for service establishment. All payments are due at the time of service, supplementation is not included in the cost of consultation. We accept cash, and credit cards. Please be familiar with your insurance carriers or Federal Spending Account reimbursement procedures, as we will gladly provide diagnostic and procedure codes, and supporting documentation to help you receive maximum reimbursement when filing a claim. TIMM does not accept payment from insurance companies and does not file claims for payment.

To be Recommended Medicinal Marijuana in
The District of Columbia
Patients must have a
Qualifying medical condition:
- Human Immunodeficiency Virus (HIV)
- Acquired Immune Deficiency Syndrome (AIDS)
- Glaucoma
- Conditions characterized by severe and persistent muscle spasms, such as multiple sclerosis
- Cancer
Qualifying medical treatment:
- Chemotherapy
- Use of azidothymidine or protease inhibitors
- Radiotherapy
A Bona Fide Doctor-Patient Relationship
Must Include
800.1 A physician who is licensed in good standing to practice medicine or osteopathy in the District of Columbia may recommend the use of medical marijuana to a qualifying patient if the physician:
(a) Is in a bona fide physician-patient relationship with the qualifying patient;
(b) Has Completed a full assessment of the patients medical history and current medical condition, including a personal physical examination, not more than ninety (90) days prior to making the recommendation;
(c) Has responsibility for the ongoing care and treatment of the patient, provided that such ongoing treatment shall not be limited to or for the primary purpose of the provision of medical marijuana use or consultation solely for that purpose;
(d) Makes the recommendation based upon the physician's assessment of the qualifying patients:
(1) Medical history;
(2) Current medical condition; and
(3) A review of other approved medications and treatments that might provide the qualifying patient with relief from a qualifying medical condition or the side effects of a qualifying medical treatment.
